How Do I Get a Library Card?
Acceptable Forms of Identification for Library Card Applications
State issued Identification with a photograph plus one of the following:
- Both the parent/minor applicant must be present at the time of application.
- The parent/guardian must present valid identification and proof of residency. (See acceptable forms of identification for library card applications listed above.)
- The parent/guardian must present "proof of guardianship." Acceptable verification for "proof of guardianship" includes one of the following for the minor child:
- Birth Certificate of child
- Social Security card of the child
- Current school ID or report card
- Learner's permit or driver's license

Immunization records
Residents of Union County, property owners, teachers and students who attend Union County schools or New Albany Schools; and police and fire department staff are eligible for a free Library Card.
Borrowers with lost or stolen Library cards should report this to any Library Check Out Desk immediately to avoid possible misuse of the card.
Borrowers with lost Library cards may obtain a replacement by applying in person at any Library Check Out Desk; there is a $1.00 fee for a new card, payable at issue. Identification showing name and current address is required.
